Please call (888) 521-6568 and select option 3 for the Claims Department. One of our claims professionals will discuss the loss information with you and assist you in completing any necessary claims forms.

If the loss is due to a fire or theft, you will be required to obtain a copy of a report from the fire or police department, in addition to completing an Affidavit of Claim.
